By submitting this form, I grant Pez Productions Ltd the irrevocable right to film, record, edit, and use my image, voice, likeness, and name in any media (including print, digital, web, and broadcast) for corporate, community, promotional, marketing, commercial, educational, social media, or any other purposes worldwide, in perpetuity.

I understand that my image or recordings may be used in their original form or edited as needed. I waive any right to inspect or approve the final product and waive any rights to royalties or compensation arising from its use.

Pez Productions Ltd will store all footage and your information securely in accordance with UK data protection standards. You may withdraw consent at any time by contacting us. We will then cease use and remove your image from future publications where possible.
